Key Qualifications:

• Active RN license in good standing
• At least one year of recent experience in Rehab or Long-Term Care nursing
• Strong commitment to patient and employee safety
• Compassionate, adaptable, and resourceful under pressure
• Excellent communication and collaboration skills

Primary Responsibilities:

• Complete comprehensive nursing assessments within 8 hours of patient admission
• Develop and revise individualized care plans based on assessment and input from the patient/family and interdisciplinary team
• Participate in rehabilitation programs and ensure continuity of care between shifts
• Accurately document all assessments, interventions, and patient responses according to policy
• Initiate patient teaching and discharge planning upon admission
• Provide supervision and guidance for ancillary nursing staff
• Engage in team-building and promote a cohesive, positive work environment
• Maintain high standards for personal appearance, attendance, and punctuality
