Qualifications/Desired Experience:

• Active RN licensure required
• Previous experience in Rehab or Long-Term Care settings preferred
• Strong assessment, documentation, and patient education skills
• Demonstrated ability to prioritize safety—both patient and team
• Excellent communication and collaboration skills
• Commitment to ongoing professional development

Responsibilities:

• Complete comprehensive nursing assessments within 8 hours of patient admission, establishing individualized care plans
• Collaborate with the interdisciplinary team to identify teaching needs, set goals, and provide updates on patient status at regular conferences
• Deliver hands-on care, implementing and evaluating plans to achieve quality outcomes for patients and their families
• Proactively initiate discharge planning and patient/family education from day one
• Maintain accurate, timely documentation in compliance with hospital policy
• Uphold personal and patient safety by adhering to best practices and reporting hazards promptly
• Supervise and guide ancillary staff, fostering teamwork, efficiency, and a positive work environment
• Communicate effectively across departments and leadership to ensure continuity of care and support unit needs
• Take charge responsibilities as assigned post-orientation
