Qualifications/Experience:
- Active RN license in good standing
- Experience in rehab or long-term care (LTC) nursing
- Familiarity with patient safety protocols and infection control
- Competency in conducting comprehensive nursing assessments
- Strong teamwork, communication, and leadership skills
Key Responsibilities:
- Complete nursing assessments within 8 hours of patient admission to lay the foundation for personalized care plans
- Identify patient/family learning needs; coordinate and document teaching and discharge planning
- Ensure timely and accurate documentation of interventions and patient responses per policy
- Participate in interdisciplinary rehabilitation programs, delivering high-quality patient care
- Maintain open communication with attending physicians and charge nurses to ensure continuity and safety
- Supervise, train, and delegate tasks to ancillary nursing staff, fostering a positive and efficient workplace
- Uphold best practices for personal and patient safety, proactively reporting and addressing hazards
