Key Responsibilities:

• Complete comprehensive nursing assessments on all patients within 8 hours of admission
• Develop individualized care plans in collaboration with patients, families, and the interdisciplinary team
• Participate actively in team conferences, providing patient updates and assessments
• Maintain accurate and timely documentation of assessments, interventions, and patient responses
• Educate patients and families to support rehabilitation goals and discharge planning from day one
• Foster patient and team safety, demonstrating strong awareness in safe patient handling, infection control, and environmental safety
• Communicate significant patient status changes to the care team to maintain continuity of care
• Supervise and delegate tasks to ancillary personnel, providing guidance and support
• Promote a positive team culture and participate in unit-based team-building activities
• Proactively identify personal professional development opportunities
