Key Responsibilities:
- Complete comprehensive nursing assessments within eight hours of patient admission
- Develop individualized care plans in collaboration with patients, families, and the interdisciplinary team
- Provide timely, accurate documentation of interventions and patient outcomes
- Offer direct patient care, utilizing evidence-based nursing processes to support rehabilitation
- Deliver education to patients and families from the point of admission through discharge planning
- Communicate significant changes in patient status to physicians and other care team members
- Supervise, support, and delegate to ancillary staff as appropriate
