Qualifications:

  • Active RN license required
  • Previous experience in rehab or long-term care preferred
  • Strong assessment, planning, and patient education abilities
  • Familiarity with safe patient handling, infection control, and hospital safety practices
  • Excellent communication and leadership skills

Essential Responsibilities:

  • Complete comprehensive nursing assessments within 8 hours of patient admission to guide individualized care planning
  • Identify patient teaching needs, develop goals, and collaborate with interdisciplinary teams and families
  • Provide timely, accurate documentation of all assessments, interventions, and patient responses
  • Participate in, and contribute to, patient rehabilitation programs utilizing the nursing process for optimal outcomes
  • Proactively plan for discharge from day one and initiate patient and family education accordingly
  • Communicate changes in patient status and pertinent information to ensure care continuity and safety
  • Direct, train, and supervise support staff; effectively delegate tasks
  • Promote an environment of safety and teamwork, participating in unit team-building and safety initiatives
  • Monitor supplies, support cost-effective care, and maintain open channels with management for service needs
