Qualifications and Experience:

  • Valid and unrestricted RN license
  • Previous experience in rehabilitation or long-term care settings preferred
  • Solid clinical assessment, documentation, and patient communication skills
  • Proficiency in administering medications, wound care, and mobility support
  • Ability to work effectively in a multidisciplinary team
  • Compassionate approach to patient care, with strong attention to detail
  • Current BLS certification

Key Responsibilities:

  • Provide direct nursing care to residents and patients, following established care plans
  • Monitor patient progress and communicate significant changes to the healthcare team
  • Collaborate with therapists, social workers, and support staff to deliver holistic rehabilitation services
  • Educate residents and families about treatment plans and lifestyle adjustments
  • Maintain accurate records and ensure all documentation meets regulatory standards
  • Assist in the development and updating of care plans according to patient needs
  • Respond promptly to emergencies and implement appropriate interventions
