Registered Nurse - Rehabilitation, (RN) in West Des Moines, IA

Qualifications and Experience:

• Active and unencumbered RN license
• Minimum one year of recent experience in rehabilitation or long-term care settings
• Strong assessment, care planning, and clinical skills
• Professional communication skills and the ability to work collaboratively with multi-disciplinary teams
• Knowledge of regulatory standards and compliance specific to long-term care and rehabilitation
• BLS certification required

Key Responsibilities:

• Conduct patient assessments and contribute to the development and implementation of individualized care plans
• Monitor patient progress, accurately document care, and communicate changes in condition to the healthcare team and family members as appropriate
• Administer medications and perform nursing interventions in accordance with established protocols and patient needs
• Serve as an advocate for patient dignity, safety, and comfort while promoting rehabilitation goals
• Educate patients and their families on care routines, therapy goals, and discharge planning
• Collaborate with physical and occupational therapists, social workers, and other providers to ensure seamless patient care
• Uphold infection control and patient safety standards throughout all aspects of care
