Qualifications & Experience:
- Active RN license with a focus on rehabilitation or long-term care preferred
- Experience in critical illness, rehab, or long-term care nursing strongly desired
- Strong assessment, care planning, and patient education skills
- Ability to work autonomously and collaboratively in an interdisciplinary team
- Clear, timely communicational abilities for comprehensive documentation and team updates
Responsibilities:
- Conduct comprehensive assessments within 8 hours of patient admission, crafting individualized care plans
- Collaborate with patients, families, and team members to identify educational needs and care goals
- Maintain accurate, timely documentation of interventions and patient responses as per hospital policy
- Take ownership of direct patient care using the nursing process to drive quality outcomes
- Initiate and guide discharge planning from the day of admission
- Communicate key status changes to team members to support continuity and safety
- Delegate tasks effectively and provide supervision and training for ancillary staff
- Create a positive, safe work environment, proactively adhere to safety protocols, and participate in team-building initiatives
- Pursue ongoing professional development through self-study and continuing education
