Schedule/Hours:

Qualifications & Experience:

• Active RN license with a focus on rehabilitation or long-term care preferred
• Experience in critical illness, rehab, or long-term care nursing strongly desired
• Strong assessment, care planning, and patient education skills
• Ability to work autonomously and collaboratively in an interdisciplinary team
• Clear, timely communicational abilities for comprehensive documentation and team updates

Responsibilities:

• Conduct comprehensive assessments within 8 hours of patient admission, crafting individualized care plans
• Collaborate with patients, families, and team members to identify educational needs and care goals
• Maintain accurate, timely documentation of interventions and patient responses as per hospital policy
• Take ownership of direct patient care using the nursing process to drive quality outcomes
• Initiate and guide discharge planning from the day of admission
• Communicate key status changes to team members to support continuity and safety
• Delegate tasks effectively and provide supervision and training for ancillary staff
• Create a positive, safe work environment, proactively adhere to safety protocols, and participate in team-building initiatives
• Pursue ongoing professional development through self-study and continuing education
