Responsibilities:

• Deliver comprehensive nursing care to patients transitioning from intensive care to stable environments
• Monitor, assess, and record patient vital signs and progress, identifying and reacting to emergent needs
• Administer medications and perform advanced nursing interventions per physician orders and protocols
• Collaborate with physicians, therapists, and other clinicians to develop and implement effective patient care plans
• Educate patients and families regarding care goals, procedures, and post-discharge instructions
• Maintain accurate documentation in compliance with healthcare policies and regulatory standards
