Job Title: Nurse Navigator RN
Position Summary:
The Nurse Navigator RN position offers a dynamic opportunity to provide comprehensive services to patients across the healthcare continuum. This role encompasses elements of a registered nurse, case manager, and patient liaison, allowing for autonomy and prioritization within a supportive environment.
As a Nurse Navigator RN, you will leverage your advanced knowledge and specialized skills to oversee clinical, quality, and cost-effective outcomes for our patient population.
Responsibilities:
- Organize patient care through prioritization, delegation, delivery, and ongoing evaluation.
- Employ critical thinking skills to make informed decisions regarding patient care, incorporating relevant standards, resources, and data.
- Utilize nursing education and experience to deliver care using the nursing process according to hospital policies and protocols.
- Collaborate with the nursing team to enhance quality of care, patient experience, and cost reduction.
- Create a learning environment for patients/families, nursing staff, and other healthcare team members.
- Advocate for ethical and holistic care, promoting patient autonomy, dignity, and health across diverse backgrounds.
Requirements:
- Bachelor's degree in Nursing, or obtained within five (5) years from initial hire date; RN with 15+ years of service may be considered in lieu of education agreement.
- Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred.
- Current Registered Nurse license in Washington State or Multistate License endorsement (MLS).
- OCN certification highly desirable.
- Three to five (3-5) years of recent clinical experience in area of specialty required.
